Compliance Agreement Procedure for Mitsubishi Lab Cell Line

The following procedure must be completed before we can ship the cell line to you. The Compliance Agreement form must be signed by the PI of your lab as all orders are shipped to the Lab PI, and we may only ship this line to people that have completed the compliance agreement process. The agreement is the same for not-for-profit and for for-profit institutions. No institutional signature is required. If you have questions about the agreement, please contact Dr. Ryu Ueda (rueda at lab.nig.ac.jp).

Please download the Compliance Agreement form and complete the following steps.

• Download the Compliance Agreement form.

• Make two (2) copies of the form.

• Print or type the name of the lab PI, the name of the company or institution, and the names of the cell lines of interest. If you fail to print clearly, we may enter the information incorrectly, and you will be unable to order your desired lines. You may enter multiple lines in a single form.

• The lab PI must sign his/her name and title in the space provided.

• Send one signed copy, for the use of MITILS, to:

• Email one signed copy to the DGRC at dgrc@iu.edu. Once we have received your form, we will enter it into our system. You will then be able to place your order.
